What Is EECP? The Scientific Foundation of Surgery-Free Heart Treatment

EECP (Enhanced External Counterpulsation) is an FDA-approved, surgery-free and incision-free treatment for patients with coronary artery disease and chest pain (angina). On this page, we take a detailed look at what EECP is, how it works, and its scientific foundations.

What Is EECP (Enhanced External Counterpulsation)?

EECP, known in Turkish as Enhanced External Counterpulsation, is a treatment method backed by decades of clinical research, designed to increase blood flow to the heart muscle in people with coronary artery disease — without surgery, incisions, or anesthesia. During treatment, special pneumatic cuffs wrapped around the legs (calves, thighs, and hips) inflate and deflate in sync with the patient's ECG rhythm.

This rhythmic, controlled pressure increases blood flow to the coronary arteries when the heart relaxes (diastole), and reduces the workload on the heart when it contracts (systole). As a result, the heart pumps more blood with less effort, and over time the body develops new small blood vessels around blocked or narrowed arteries. This natural process is referred to in the literature as "the natural bypass effect" or collateral circulation .

EECP offers a safe, evidence-based alternative for patients at high surgical risk, those whose symptoms persist after coronary bypass or angioplasty, those who continue to experience chest pain despite medication, or those who are not suitable candidates for surgery. At EECP Kıbrıs, this treatment is administered under the direct supervision of Prof. Dr. Osman Beton, using equipment that meets international standards.

MECHANISM

How Does EECP Treatment Affect the Body?

The treatment works through four complementary physiological mechanisms:

1 Diastolic Augmentation As the heart relaxes, the cuffs inflate sequentially from the ankles to the hips, increasing blood pressure to the coronary arteries. 2 Systolic Unloading As the heart contracts, the cuffs deflate suddenly, reducing the effort and oxygen demand required for the heart to pump blood. 3 Vascular Wall Stimulation The increased blood flow velocity stimulates the cells lining the inner vessel wall (endothelium), triggering the release of natural substances that support vessel dilation. 4 Collateral Vessel Development Through regular sessions, the body develops new small blood vessels around blocked arteries, creating alternative blood pathways to the heart.
SCIENTIFIC BASIS

The Scientific History and Approvals of EECP

EECP is the product of decades of research into heart failure and coronary artery disease. The method has been reviewed by relevant health authorities for use in the treatment of chronic stable angina (chest pain) and has become a guideline-recognized treatment option used in thousands of cardiology centers worldwide.

To date, worldwide, more than 350,000 patients, in more than 40 countries have received EECP treatment. Clinical observations report that a significant proportion of patients who complete treatment experience a reduction in the frequency of chest pain, an increase in exercise capacity, and a marked improvement in quality of life. At EECP Kıbrıs, we conduct our patients' treatment in line with internationally recognized protocols, drawing on Prof. Dr. Osman Beton's more than 20 years of clinical experience.

Important note: The information on this page is for general informational purposes only and does not substitute for medical advice. Your suitability for EECP treatment is determined by evaluating your current heart condition, previous treatments, and general health status. Always consult a cardiology specialist for a definitive assessment.

CLINICAL RESULTS

EECP Treatment in the Scientific Literature

EECP is a treatment method that has been studied in the international cardiology literature for years and is supported by clinical registry studies. Some findings reported in international patient registries are as follows:

%75–80

Reported reduction in chest pain frequency in international patient registries

1–3

Range of reported duration of benefit, in years (varies by individual)

35

Sessions in the standard treatment program (over 7 weeks)

0

Need for anesthesia, incisions, or recovery time

*The rates shown are compiled from international clinical registry studies; individual results may vary based on the patient's overall health. A physician evaluation is required prior to treatment.

SUITABILITY

Who May Be a Good Candidate?

Patients for whom bypass surgery has been recommended but who do not want or cannot undergo surgery Patients with persistent chest pain (angina) after stent or bypass Chronic angina patients whose symptoms continue despite medication Patients at high surgical risk due to advanced age or additional health conditions IMPORTANT CONSIDERATIONS

Who May Not Be a Good Candidate?

Those with uncontrolled arrhythmia or serious heart valve disease Those with serious venous disease or a history of blood clots in the legs Patients with uncontrolled high blood pressure Those who have recently had a heart attack (in the acute phase)

This list is not exhaustive. A definitive suitability assessment can only be made after a comprehensive examination.

FREQUENTLY ASKED QUESTIONS

Common Questions About EECP

Is EECP treatment painful?

Most patients find the treatment comfortable. The inflation and deflation of the cuffs can create a squeezing sensation in the legs, but this is generally not painful. A brief adjustment period may occur during the first few sessions.

How long does an EECP session take?

A standard session takes approximately 60 minutes. The full treatment program is typically planned for 5 days a week, for a total of 35 sessions over about 7 weeks.

Does EECP treatment have any side effects?

EECP is generally considered a safe treatment. Some patients may experience mild redness or fatigue in the legs during the first days of treatment; these symptoms usually resolve on their own within a short time.

When do results appear after treatment?

Some patients begin to notice a reduction in chest pain around the midpoint of treatment; the most noticeable improvement is usually assessed after treatment is completed. Results can vary from person to person.

Does EECP replace medication?

No. EECP does not replace the medication prescribed by your doctor; it is generally applied as an addition to your current treatment. You should not stop your medications without consulting your doctor.
